Bibliography citation style doesn't match the department's specification. This alone causes about 35% of the rejections we see. Sometimes it's APA vs. APA 7th edition. Sometimes it's author-year vs. numeric. Small differences, big consequences.
Front matter elements are missing or in the wrong order. Declaration of authorship, abstract placement, committee signature page - every university has a specific sequence, and many LaTeX templates don't enforce it automatically.
Inconsistent heading numbering after appendices. LaTeX's \appendix command resets counters in ways that break cross-references. Chapter 5 suddenly becomes "Appendix A" in the TOC but "Chapter 6" in the body text.
Table of contents depth doesn't match university requirements. Some want 3 levels, some want 2, some want subsections only in certain chapters. The \setcounter{tocdepth} command is the fix, but it needs to be applied selectively.
Page numbering switches incorrectly between roman and arabic numerals at the front/main matter boundary. The \frontmatter and \mainmatter commands handle this in book classes, but many university templates use article or report classes where you have to manage it manually.
